Waste incident report form. A printable incident report for spills, escapes, fly-tipping found, fires and anything else that has to be recorded and sometimes reported.
This is the paper version. WasteOptix does the same job on a phone, with the record filed against the round and the vehicle automatically.

What is on the sheet
- What happened, where, when and who was involved
- The waste concerned, and the note it was moving on if it was in transit
- Immediate action: containment, clean-up, people kept away, services shut off
- Environmental impact — watercourse, drain, soil, air — and what was done about each
- Who was notified, including the regulator where applicable, and the reference received
- Root cause, actions with owners and dates, and what changed as a result
Why this record matters
The incidents that matter in this trade are the ones where waste ends up somewhere it should not be. A spill into a drain is a different event from a spill onto concrete, and the first few minutes decide which one you are dealing with — so containment is at the top of the form rather than at the end of a narrative.
Whether the regulator has to be told depends on what happened and what it reached, and this form does not answer that. It asks who considered the question, when, and what they decided — which is the thing an investigation looks for and the thing nobody records.
Fly-tipping found on a round is on the same form on purpose. It is somebody else's offence and it becomes your problem the moment your crew touches it, and the record of what was found, where and when is what keeps it somebody else's.
Root cause is separated from immediate cause because the immediate cause is rarely useful. 'Drum leaked' stops the enquiry; why an unlabelled drum was in a general waste container, and whether that customer has done it before, is where the next incident gets prevented.
