Rejected load record. A printable record for a load a site has turned away — what was in it, why it was refused, where it went instead and the paperwork that has to follow it.
This is the paper version. WasteOptix does the same job on a phone, with the record filed against the round and the vehicle automatically.

What is on the sheet
- Vehicle, driver, round and the transfer or consignment note the load moved on
- The receiving site, who refused it, and the reason they gave
- What was actually found in the load, described rather than categorised
- Where the load went instead, and the note covering that movement
- Who was told: the producer, the customer, the office and the regulator where applicable
- Root cause and the action taken, so the same customer does not fill the same bin the same way next week
Why this record matters
A rejected load is a compliance event, not just a bad day. The waste is now somewhere it was not documented to go, there is a movement with no note covering it unless somebody writes one, and the original description on the transfer note has been demonstrated to be wrong.
The second note is the thing most often missed. A load that leaves a site it was refused at is a new transfer, and driving it back to the depot or on to another site without covering it is the breach that follows the first one.
Describing what was found matters more than the site's reason. 'Contaminated' is what the weighbridge says; 'two drums of unknown liquid under the skip waste' is what tells you which customer and which process, and whether this is a conversation or a serious problem.
The root cause line is where this record stops being an incident log. Most rejections trace to a description written for the contract rather than the waste, or a customer putting something in a container nobody told them not to — and both are fixable before the next collection.
