Round sheet template. A printable round sheet for a collection day — the stops in order, what was lifted at each, what was missed and why, and the weights that come back off the weighbridge.
This is the paper version. WasteOptix does the same job on a phone, with the record filed against the round and the vehicle automatically.

What is on the sheet
- Round, date, vehicle, driver and crew
- A row per stop: customer, address, container type and the service due
- Lifted, part-lifted or missed, with a reason code written in words rather than a number
- Contamination found, and whether the load was taken anyway
- Access problems, parked cars, locked gates and anything that will recur next week
- Weighbridge tickets, tip sites used, and a sign-off from the driver
Why this record matters
A round sheet is the only record of what actually happened on a route, and it is the document that answers the two questions customers ring about: did you come, and why did you not empty it. A round without a sheet is a round where the office has to take somebody's word for it a fortnight later.
Reasons in words rather than codes is a deliberate choice. 'Access' covers a parked car, a locked gate, a skip in the way and a dog, and only one of those is going to be fixed by ringing the customer. A code that collapses them is a code that hides the one thing you could do something about.
Contamination gets its own column because of what it costs downstream. A contaminated load taken anyway is a rejected load at the tip or a charge on the ticket, and the time to record it is at the bin, with the crew looking at it, not when the invoice is queried.
The recurring-problem line is the one that earns its keep over months. The same locked gate on the same Tuesday is a commercial conversation, and it never happens if each week's sheet goes into a folder on its own.
